In order to provide a safe and secure environment for our Church's children and youth, we believe it is necessary to include the following questions as a part of our application process. All information will be kept confidential and will not be disclosed unless required by law. Answering yes to any of the questions may not necessarily prevent you from volunteering within the church. A meeting will be arranged with a pastor so that you may discuss the circumstances. Thank you in advance for your understanding.

PHOTOGRAPHIC RELEASE

We try to capture the moments in Lakeview Kids and Youth and share those memories with our community via Instagram and Facebook. Our team always strives to be respectful and honouring of our volunteers and students whenever we are making any photographs or videos public.

I hereby grant permission to the leaders of Lakeview Kids and Youth to photograph and/or video record for program and general church use including, but not limited to, highlight videos, photo directories, social media, and website galleries. *

RELEASE OF INFORMATION AND DECLARATION OF INTENT

I hereby acknowledge that this information contained in this application is correct to the best of my knowledge. I hereby give Lakeview Church consent to verify the information provided by me in this application form and to contact the references and current employer listed above and to obtain and verify any information from them, and any other persons Lakeview Church determines might be able to provide relevant information to my application.

I grant Lakeview Church permission to perform a Criminal Record Check for the purpose of my protection against any false allegations and for the protection of those I serve. I consent to such an investigation with the understanding that the results will be kept in extreme confidentiality.

I understand that if Lakeview Church approves my volunteer application and later determines, in its' discretion, at any time that I am not suitable for volunteer service in Lakeview Church or for the volunteer position for which I am applying, Lakeview Church may terminate my volunteer service or position for any reason without advance notice.

I will at all times cooperate fully with the staff of Lakeview Church in the fulfillment of my duties and will keep all confidential information I encounter in my role as a volunteer, confidential.

If at any time I determine that for any reason I am unable to support or adhere to or follow the policies, procedures or doctrine of Lakeview Church, I will inform Lakeview Church and will resign from my volunteer position.

I further agree to adhere to the Child Protection Policy as adopted by Lakeview Church.
